Annual Health and Medical Record

You can download the official forms directly from the Scouting America Annual Health and Medical Record Page:

Form A/B (required for all Scouting activities)

Form C (required for Scouting activities lasting more than 72 hours)

NOTE: It is the responsibility of each parent to be sure all forms are complete and timely.

1. Use the fillable PDF of the medical form:

   • Forms A/B (for all activities)

   • Form C (for events longer than 72 hours)

2. Print it out & sign it.
   
   

3. Submit TWO HARD COPIES of completed forms A, B, C, and a copy of both sides of your insurance card.
   
   

4. Plan ahead and have forms turned in well before your trip.
   
   

5. To request exemption from medical care and treatment, fill out these two forms and submit TWO HARD COPIES:
   BSA Medical Care and Treatment Exemption Request
   
   Immunization Exemption Request Form
